Ash picks off just where the original Galerians left off with the continuing saga of the Galerians. Five hundred years into the future, an extremely clever mother computer named Dorothy was tasked with supervising the lives of humanity. However, the computer’s intelligence instructed her that humans had to be eradicated since they were imperfect and feeble beings and that instead, a race of super-humans called the Galerians had to be created. But in a tense battle, Rion, the first game’s hero, was able to defeat Dorothy and take her down. Now, Rion has been given a second chance at life, and she finds out that Dorothy could run a devastating program just before she passed away. Once more, Rion must confront and defeat the Galerians, Dorothy’s inventions.

The gameplay is very similar to the game that came before it, and it features a typical survival horror structure supplemented with the protagonist’s unique skills. Rion is capable of deploying a variety of psychic abilities to combat his foes. When he uses a technique more frequently and takes more damage, he becomes more “addicted” to using that method. When the addiction meter reaches its maximum, his hit points are rapidly reduced, and he can no longer walk normally. As is the case in most survival horror games, the main character places significant importance on things that can heal or restore health. Like that an RPG, enemies will also drop goods that have the potential to increase Rion’s maximum hit points or make his weapons more powerful.
